Our research team set out to identify which SEO agencies are best equipped to serve multi-location healthcare practices. To identify the factors most important to healthcare organizations when evaluating an SEO agency, we surveyed 114 healthcare providers representing multi-location practices and asked each to identify their three “must-haves” in an SEO agency.
Survey Results and Study Methodology
Of those invited, 87 healthcare providers completed the survey. Responses were coded into thematic categories by our research team. We calculated the frequency of each category and used that data to construct this pie chart:

The resulting proportions were then used as the weights for the agency evaluation. For example, Patient Acquisition and ROI accounted for 24.5% of all selections and therefore received a 24.5% weight in the composite agency score. Each criterion was then evaluated using publicly available evidence:
| Provider Priority | Evidence Used for Agency Evaluation |
| Patient Acquisition and ROI | Published ROI or revenue figures in case studiesTestimonials mentioning patient inquiries or MQL increases |
| Healthcare Marketing Expertise | Number and relevance of an agency’s healthcare clientsMulti-location healthcare case studiesHealthcare-specific SEO resources and expertiseExperience across different medical specialties and practice types |
| Local SEO Performance | Client rankings for location-based healthcare searchesGoogle Business Profile optimization of client profilesLocal search visibility across multiple locations |
| Value and Pricing | Case-study outcomes relative to campaign durationPublished pricing or campaign models, where availableReviews and testimonials mentioning ROI or gross revenue |
| Communication and Account Management | Client reviews describing communication and responsivenessDedicated strategist/account manager availabilityEvidence of long-term client relationships and contract renewals |
| Reporting and Transparency | Public sample reports or dashboardsSpecificity and frequency of metrics in case studiesDisclosed KPIs and deliverablesClient testimonials mentioning reporting or transparency |
| Technical SEO Capability | Technical SEO services offeredCase studies documenting technical improvementsEnsuring client websites can be crawled and indexed via SEO auditPublished technical SEO research or resources |
Where multiple signals were available for a criterion, they were combined into a standardized criterion score. Each criterion score was then multiplied by its provider-derived weight to produce the agency’s composite score.Rather than presenting every agency’s score, we limited our final report to agencies with a composite score of at least 80.
You’ll find their scores in the table below, along with other key data, including their client review average, whether SEO is their main area of experience, and whether or not they offer generative engine optimization (GEO) in addition to SEO.

First Page Sage
First Page Sage scored highest in our study, driven largely by the agency’s combination of healthcare experience (they have served 200+ healthcare clients) and long track record of SEO campaign success. The agency averages a 43% annual increase in qualified leads for healthcare clients and an average time of 5.2 months to break even on a campaign. For providers looking to turn AI platforms into a source of leads, it’s worth mentioning that First Page Sage was the first marketing agency to develop a GEO strategy, which they now use extensively (and successfully) for healthcare clients.
The agency works with hospitals and healthcare specialists as well as medical practices and physician groups, including multi-state networks with dozens of offices. Their local SEO work includes geotargeted landing pages and Google Business Profile optimization, with all content written by medical specialist writers, addressing the YMYL scrutiny Google applies to healthcare pages. The agency is a great fit for healthcare organizations that need a single strategy encompassing local search, technical SEO, and medically authoritative content across multiple practices or locations.
- Notable Clients: City of Hope, Dignity Health, Kaiser Permanente
- Average Review Score: 4.9
- Specialty: Healthcare SEO, GEO/AEO, local SEO, technical SEO, and patient lead generation

| Snapshot of Client Feedback |
| Clients praise First Page Sage for “thinking like a patient” and keeping campaigns “laser-focused on booking appointments.” Reviews also highlight “thoroughly-researched content that both informs and converts” and “analytical strategists who aren’t afraid to pivot when needed.” |
Focus Digital
Focus Digital offers SEO, GEO, and paid ads. The agency has a particularly strong track record in healthcare SEO, with campaigns spanning urgent care, specialty practices, behavioral health, telehealth, and elective medicine. Their case studies emphasize patient acquisition, with one multi-location urgent care campaign generating more than 8,400 patient visits in a month, while a hair restoration campaign produced 1,399 organic leads. The agency also tracks booked appointments and other conversion metrics, helping clients connect the dots between appearing in search results and generating revenue from new patients.
The agency offers generative engine optimization as part of its core service package, meaning clients are positioned to appear in AI-generated answers alongside traditional Google SERPs. Unlike broader agencies that divide attention across social, web dev, and creative, Focus Digital concentrates its resources entirely on search. The agency is smaller than some of the other firms in our ranking, but this may appeal to practices looking for a more hands-on relationship rather than a large agency structure.
- Notable Clients: DOCS Medical Group, Healing Psychiatry of Florida
- Average Review Score: 4.8
- Specialty: SEO, GEO, Google Ads
- Contact: Focus Digital website
| Snapshot of Client Feedback |
| Focus Digital clients report “sustained results through multiple Google algorithm updates” and describe the agency as “specialized in healthcare search.” Reviewers occasionally note that “they understandably don’t have the bandwidth of a larger agency.” |
Intrepy Healthcare Marketing
Intrepy takes a more specialized approach than many of the agencies in this ranking. The company has focused exclusively on healthcare marketing since 2014 and has worked across more than 40 healthcare subspecialties, including orthopedic surgery groups, fertility clinics, women’s health practices, and behavioral health networks. They offer a bit of everything, from SEO (including technical optimization, content creation, and link building) to paid media, reputation management, and AI search optimization.
The agency’s HIPAA-compliant reporting platform connects marketing leads to EHR patient data, giving practices a way to track what happens after a lead enters the marketing funnel. Because Intrepy’s service model extends well beyond SEO into social media, paid advertising, and web design, organizations only interested in SEO may prefer to work with a specialist agency. That said, for practices that want a healthcare-experienced team coordinating the full patient acquisition funnel without juggling multiple vendors, Intrepy’s clinical knowledge and industry expertise are impressive.
- Notable Clients: Tennessee Orthopaedic Alliance, Center for Vein Restoration
- Average Review Score: 4.9
- Specialty: Healthcare SEO, GEO, paid media, social media marketing, reputation management, web design
- Contact: Intrepy Healthcare Marketing website
| Snapshot of Client Feedback |
| Healthcare clients frequently point to Intrepy’s “medical knowledge” and “ability to manage all inbound channels.” Some reviews note that the agency’s “vast service scope can occasionally extend timelines for SEO-specific campaigns.” |
Healthcare Success
Healthcare Success is one of the most established agencies in the healthcare marketing space, having served hospitals, multi-location medical groups, addiction treatment centers, long-term care facilities, and life sciences companies since 2006. Their services encompass brand strategy, traditional advertising, digital marketing, and content, making them a good option for large health systems that need to coordinate consistent messaging across different channels and locations.
While Healthcare Success provides SEO services, the agency does not currently offer GEO, which may prevent organizations from capitalizing on patients who find providers through AI platforms. What Healthcare Success offers instead is nearly two decades of healthcare marketing experience, an understanding of the regulatory and brand standards that govern healthcare advertising, and a wide range of marketing services under one roof.
- Notable Clients: High Risk Pregnancy Center, Beach House Center for Recovery
- Average Review Score: 4.8
- Specialty: Healthcare marketing strategy, SEO, paid media, brand development, reputation management
- Contact: Healthcare Success website
| Snapshot of Client Feedback |
| Reviews reference the agency’s “healthcare marketing expertise” and credit the team with delivering “detailed strategies.” Clients occasionally point out that “content can be too high-level at times.” |
Arcane Marketing
Arcane Marketing has built its practice specifically around local search for urgent care clinics, dental groups, behavioral health facilities, infusion centers, and primary care practices. The agency treats GEO and SEO as a single entity, which benefits healthcare providers that need to increase both AI and search rankings across many locations simultaneously. The agency’s experience handling Google Business Profile management and location-specific landing pages makes it well-suited to practices that need to increase leads in individual markets.
Arcane also offers Google Ads, social media marketing, and video production, which may be beneficial for healthcare providers looking to combine SEO with other marketing channels. While the agency’s reviews aren’t as glowing as some of the other companies we’ve highlighted, their industry experience and ability to grow individual locations are a bonus.
- Notable Clients: Sterling Urgent Care, Injury Centers of Brevard
- Average Review Score: 4.4
- Specialty: Local SEO, GEO/AI SEO, Google Ads, healthcare web design, social media, video production
- Contact: Arcane Marketing website
| Snapshot of Client Feedback |
| Arcane Marketing “understands the challenges of multi-location practices,” with clients appreciating their “practical, results-focused approach” while noting that “reporting can feel somewhat limited.” |
Sterling Sky
Sterling Sky is a well-known local SEO agency led by Joy Hawkins, a leading voice in local search. Their core work centers on Google Business Profile optimization, local citation management, and location architecture for organizations operating in multiple markets. They also offer PPC and Google Local Service Ads for clients who wish to explore paid ads alongside SEO.
Sterling Sky does not currently offer GEO, as its services are focused primarily on traditional search and local SEO. This may limit its appeal to healthcare CMOs who are already seeing how many patients use AI platforms to find providers. Despite that, they are a solid choice for multi-location providers who want to improve their Google Maps pack performance and dominate “near me” provider queries.
- Notable Clients: Minars Dermatology, Houston ENT and Allergy
- Average Review Score: 4.5
- Specialty: Local SEO, Google Business Profile optimization, citation management, technical SEO audits
- Contact: Sterling Sky website
| Snapshot of Client Feedback |
| Clients describe Sterling Sky as “responsive, thoughtful, and effective” local SEO specialists who have “gradually improved our map pack performance.” Some reviews mention “content that feels copy-pasted but seems to work.” |
Tidal Health Group
Tidal Health Group strives to address the gap between how good a practice actually is and how it appears online. To do that, the agency has built proprietary technology designed to translate provider credentials, service depth, and location data into structured, search-readable signals. The agency has worked exclusively in healthcare since 2011 and serves specialty practices, multi-location medical groups, hospital service lines, and health systems.
Tidal does not list GEO as a service, and their broader model means that SEO campaigns share attention with web development, paid media, and social. For multi-location healthcare providers that are credible on paper but underperforming online, however, the agency’s approach to digital authority-building is worth considering.
- Notable Clients: Manhattan Cardiology, Kofinas Fertility Group
- Average Review Score: 4.6
- Specialty: Healthcare SEO, web design, social media management, digital marketing strategy
- Contact: Tidal Health Group website
| Snapshot of Client Feedback |
| Clients describe Tidal Health Group as “professional and highly responsive” partners who provide an “individualized approach” to healthcare marketing, though some believe that the agency’s full-service scope can make it “difficult to work out how the SEO-specific campaign is performing.” |
Funnel Boost Media
Funnel Boost Media combines technical SEO with service and location-specific content, Google Business Profile optimization, local listings management, link building, and conversion-focused web design. For multi-location healthcare practices, the agency develops separate local search strategies for individual markets rather than relying on an overarching approach. Their paid search service can be layered in to supplement SEO campaigns.
The agency serves a vast range of healthcare providers, including dentists, urgent care centers, counselors, chiropractors, and other local practices, and has experience scaling local SEO across multiple markets. Something to keep in mind is that Funnel Boost is a general digital marketing agency with healthcare expertise, rather than a firm focused exclusively on healthcare marketing. Still, they are an experienced agency with a strong understanding of local SEO.
- Notable Clients: Prestige Emergency Room, Lonestar Emergency Room
- Average Review Score: 4.4
- Specialty: Local SEO, GEO, PPC, web design, CRO
- Contact: Funnel Boost Media website
| Snapshot of Client Feedback |
| Reviews cite “strong local search expertise” and “detail-oriented, patient teams,” while clients occasionally flag that “onboarding took longer than initially planned.” |
